Information about this clinical experience:

During this US clinical experience in nephrology, you will be under the guidance of a board-certified nephrologist. The preceptor rounds at multiple hospitals, including Houston Methodist Hospital, Baylor St. Luke's Hospital, Memorial Hermann Southwest Hospital, Kindred Hospital, and Cornerstone Hospital. 


In this experience, you will be exposed to a wide range of patients with kidney disorders. These will include primary kidney disorders as well as systemic conditions affecting the kidney. You will learn how to evaluate and treat patients with AKI, CKD, ESRD, electrolyte abnormalities, and acid-base disturbances.


In the future as a resident, many of your hospitalized patients will have AKI. In fact, the rate of hospitalizations with AKI has been increasing. Over 50% of patients hospitalized for sepsis have AKI. While AKI is less common among patients hospitalized for non-sepsis infection and for cancer, over 20% of patients with these conditions will have AKI. As AKI is associated with higher incidence of chronic kidney disease and mortality, it is essential for physicians to have a strong understanding of how to evaluate and treat this problem.

During your experience, you will learn the following U.S. medical practices under the preceptor’s supervision:


  • Taking patient histories.

  • Performing physical exams under the physician's supervision.

  • Presenting patients.

  • Writing progress notes in paper charts. 

  • Have opportunities to research the literature to answer clinical questions at the point of care.

  • Spending time with other healthcare professionals to understand their roles and responsibilities.

Who should consider this rotation:

International medical graduates (IMGs) and students seeking US clinical experience. IMGs seeking careers in family medicine or internal medicine will find this rotation particularly useful since problems involving the kidney are common in inpatient and outpatient practice.

How to obtain a letter of recommendation:

The rotator should ask the preceptor for a LOR near the conclusion of the rotation. Dr. Desai has provided the physician with guidelines about best practices in letter writing that meet residency program requirements. In some cases, the preceptor may ask the rotator to write a letter of recommendation draft.

This rotation provides students a very good learning opportunity and gives great insight into the working life of a Cardiologist. This preceptor makes a strong effort for students to get the most out of the rotation. He encourages questions and provides thorough explanations. Although this rotation is intended as a learning opportunity for the student, he integrates you very well into his practice and makes you feel like you are a valuable member of the team. You will have the opportunity to see many patients in clinic. You will take histories and physical exams of these patients and then present them to the preceptor followed by your own assessment and plan, with subsequent constructive feedback from the preceptor. In clinic, you will also have the opportunity to perform ECGs, observe venous ablation procedures, observe and interpret treadmill stress tests, and receive an introduction on how to interpret nuclear stress tests. You will have the opportunity to round on patients 2-3x times a week in the hospital and compose progress notes about them in the electronic medical record. You will also have the opportunity to compose consultation notes in the electronic medical record about newly admitted patients. You will have the opportunity to present these patients to the preceptor and receive guidance and constructive feedback about your notes. In the hospital, you will have the opportunity to observe angiograms/angioplasties and pacemaker implantations. During your entire rotation, you will also work closely with two knowledgeable nurse practitioners who are kind, patient, and interesting in helping you become successful during your time on this rotation. I highly recommend this rotation. I am confident that you will learn a lot and enjoy your time working with this preceptor and his team.
