Hours:

Monday 8 AM to 5 PM; Tuesday 8 AM to 2 PM; Wednesday 8 AM to 5 PM; Thursday 8 AM to 2 PM; and one weekend per month when the physician is on call at the hospital. The rotator is also welcome to observe Transcranial Magnetic Stimulation (TMS) conducted by technicians in the office on Fridays (this is an optional activity; please note that the preceptor will not be in the office on Fridays).

Hours have been provided by the preceptor. Hours are subject to change at the preceptor’s discretion, but the minimum total hours will remain approximately the same.

This combined inpatient and outpatient US clinical experience will take place approximately 115 miles West of downtown Detroit. Although you will be spending time at the preceptor's outpatient practice and multiple hospitals, transportation will not be needed to travel to these locations as you will accompany the preceptor in his car.

Information about this clinical experience:

You will be under the supervision of a physician who is board-certified in psychiatry. This is a very experienced psychiatrist with decades of practice in the field. The attending physician holds a hospital appointment in the Bronson Healthcare system.

Rotators will split their time between the inpatient and outpatient settings. The preceptor is very kind and caring and is committed to developing a non-threatening environment in which students can optimally learn. Patients with a wide range of mental health conditions, including depression, anxiety, bipolar disorder, substance abuse, and PTSD. As you participate in their care, you will periodically receive feedback to guide your growth in such areas as history taking and case presentation.

You will also be able to observe patient support groups. Members of these groups will share their personal stories and struggles, giving you insight into how these groups provide emotional support and help their members cope with their illness.

This practitioner is trained in treatment modalities that patients require when standard treatments have proved unsuccessful. For example, depressed patients who have not responded to psychotherapy or medication may be treated with Transcranial Magnetic Stimulation (TMS). Rotators will have the opportunity to understand the indications for such treatment, how the procedure is conducted, and other issues related to the therapy.

In the hospital, you will accompany the preceptor to evaluate and manage inpatients on the medical-surgical ward. These are patients that are primarily under the care of specialists in other fields but have issues that require the expertise of a psychiatrist. These may include patients with psychiatric manifestations of medical or surgical illness or patients with known psychiatric conditions who are medically ill. During your psychiatry residency, you will have dedicated rotations where you will learn to care for such patients; therefore, this experience will help ready you for that responsibility. You may also have the opportunity to attend rounds with the preceptor at an inpatient psychiatric facility.

Who should consider this rotation:

International medical graduates (IMGs) and students seeking US clinical experience in psychiatry. IMGs seeking careers in family medicine or internal medicine will also find this rotation particularly useful since mental health problems are common in these specialties.

How to obtain a letter of recommendation:

The rotator should ask the preceptor for a LOR near the conclusion of the rotation. Dr. Desai has provided the physician with guidelines about best practices in letter writing that meet residency program requirements. In some cases, the preceptor may ask the rotator to write a letter of recommendation draft.
