This inpatient and outpatient US clinical experience will take place approximately 30 miles north of downtown Miami. There are two outpatient locations that are 5 miles apart. The inpatient component will take place at several hospitals and a surgery center.

Information about this clinical experience:

You will be under the supervision of a physician board-certified in both anesthesiology and pain medicine. The physician currently holds a hospital staff appointment within the HCA Health System. The preceptor’s practice is a teaching site for a local neurology residency program and medical school. In addition, the preceptor is approached from time to time by anesthesiology residents seeking to round in the hospital or spend time at the practice.


In this clinical experience, you will encounter patients with low back pain, neck pain, joint pain, neuropathy, stenosis, degenerative disc disease, herniated disc, radiculopathy, sciatica, and neuralgia among other conditions. As you evaluate these patients, you will learn how to perform a focused history and exam and select an appropriate imaging test if it is needed. You will appreciate the effects of pain on the patient's quality of life and function and understand how to best interact with patients in a compassionate and empathetic manner.


The attending has developed expertise in cutting-edge interventional pain procedures and utilizes an extensive breadth of knowledge and technical ability to provide relief to patients suffering from pain. Procedures that you may be able to observe include the following:


  • Epidural steroid injection

  • Facet joint injections

  • Knee/shoulder/hip/SI joint injections

  • Nerve blocks

  • Plasma PRP injection

  • Discectomy

  • Kyphoplasty

  • Spinal/peripheral stimulators


The inpatient component of this clinical experience will take place at several hospitals and a surgery center where the attending performs procedures. During your experience, you will learn the following U.S. medical practices under the preceptor’s supervision:


  • Taking patient histories.

  • Performing exams under the physician’s supervision.

  • Presenting cases.

  • Entering notes in the EMR (CareCloud)

  • Observing procedures.

  • Researching the literature to answer clinical questions at the point of care.

  • Spending time with other healthcare professionals to understand their roles and responsibilities.


The activities above will mostly take place in the outpatient setting. As a result of hospital policies, inpatient activities will be more observational.


Please note some part of your time in this clinical experience will be spent rotating with the physician’s colleague in the practice.

Who should consider this rotation:

International medical graduates (IMGs) and students seeking US clinical experience in anesthesiology or pain medicine.

How to obtain a letter of recommendation:

The rotator should ask the preceptor for a LOR near the conclusion of the rotation. Dr. Desai has provided the physician with guidelines about best practices in letter writing that meet residency program requirements. In some cases, the preceptor may ask the rotator to write a letter of recommendation draft.
