Inpatient/Outpatient US Clinical Experience in Family Medicine in Orlando

Information about this clinical experience:

You will be under the supervision of a board-certified family medicine physician. The preceptor holds hospital privileges at Advent Health. The physician has been involved in the education of family medicine residents from Florida Health, and these trainees sometimes rotate in the doctor's outpatient practice. The preceptor has several decades of experience as a clinician and educator.  The physician has taught international medical graduates from all over the world, and this gives the doctor's professional life considerable meaning.


You will see patients with the preceptor in the hospital and outpatient practice from 9 AM to 5 PM. The day begins at the outpatient practice where you will see patients with the preceptor in the office until Noon. From 12:30 to 1:30 PM, you will visit the hospital with the preceptor to take part in hospital rounds. The remaining part of the day will be spent in the office seeing outpatients. Please note that the typical census in the hospital is 3-4 patients.


During your experience, you will learn the following U.S. medical practices under the preceptor’s supervision:


  • Taking patient histories under supervision.

  • Performing exams under the physician's supervision.

  • Presenting patients.

  • Researching the literature to answer clinical questions at the point of care.

  • Educating patients under supervision.

  • Spending time with other healthcare professionals to understand their roles and responsibilities.


The activities above will mostly take place in the outpatient setting. As a result of hospital policies, inpatient activities will be more observational.


Please note that 40-50% of the patients are Spanish-speaking.

Who should consider this rotation:

International medical graduates (IMGs) and students seeking US clinical experience in family medicine in both the inpatient and outpatient setting.

How to obtain a letter of recommendation:

The rotator should ask the preceptor for a LOR near the conclusion of the rotation. Dr. Desai has provided the physician with guidelines about best practices in letter writing that meet residency program requirements. In some cases, the preceptor may ask the rotator to write a letter of recommendation draft.

This rotation gave me a tremendous exposure to patients from various socioeconomic backgrounds and stages of disease. I am confident that I saw more Parkinson’s patient (90 cases, from idiopathic to drug-induced and all possible manifestations of the disorder) than many of the first year residents back in my home country. The nursing facilities we visited also ranged from places situated in poor neighborhoods with lack of funding to high-end luxury facilities. The preceptor’s private clinic allows us many opportunities to interview and perform physical exams on patients which is really helpful. The inpatient psych rounds in the morning is my favorite part of the day. I got the chance to interview tons of patients for intake and by the end-of the rotation, I was able to interview new patients, diagnose, and suggest treatments within 10 minutes. I also got the chance to practice driving in the US as this rotation demands that we drive to various different places across the suburbs of Chicago. Overall, this rotation gave me the confidence that not only will I survive in the US residency training system, I will thrive in it.
