This inpatient and outpatient US clinical experience will take place in Houston, Texas (approximately 10 miles west of downtown).
Combined Inpatient & Outpatient US Clinical Experience in Internal Medicine
What you will do:
This rotation split between the inpatient and outpatient settings will be under the supervision of a board-certified internal medicine physician with years of experience educating students. The physician holds hospital appointment within the Memorial Hermann Healthcare System and is Clinical Assistant Professor of Medicine at Sam Houston University College of Osteopathic Medicine.
In the clinic, you will see patients with both acute complaints and chronic disease. You will learn how to evaluate these patients through focused history-taking and examination and synthesize the data to form an assessment and plan. During discussions with the attending, important teaching points related to these cases will be shared with you. Of note, the patient population that the preceptor cares for includes many patients with multiple comorbidities. Therefore, this experience will be an excellent opportunity for you to learn how to manage patients with complex medical and social conditions. This physician is well regarded for providing patients with personalized care and believes in taking the time necessary to listen to patients and make them feel heard.
On a typical day, 15-20 patients are seen. 70% of the patient visits are for typical problems encountered in primary care internal medicine. 30% of patients may have more specialized problems, including chronic kidney and end-stage renal disease. One day per week, the student will have the opportunity to visit a dialysis center. ​Hospital rounds will take place approximately once or twice per week depending upon the patient census in the hospital.
After a few days of observation, you will take on an increasing role in patient encounters and learn the following under the preceptor’s supervision:​

Taking patient histories under supervision.
Performing exams under the physician's supervision.
Presenting patients.
Researching the literature to answer clinical questions at the point of care.
Educating patients under supervision.
Spending time with other healthcare professionals to understand their roles and responsibilities.​

How to obtain a letter of recommendation:
The rotator should ask the preceptor for a LOR near the conclusion of the rotation. Dr. Desai has provided the physician with guidelines about best practices in letter writing that meet residency program requirements. In some cases, the preceptor may ask the rotator to write a letter of recommendation draft.
